October 2, 2026 — The NTSB’s preliminary report on the fatal 21 Air (2I) Boeing 767 overrun at Miami describes a fast, long landing with no speed brakes or reverse thrust, and a controller who never passed on the tailwind.

What happened

21 Air Flight 7598 was flying cargo for Amazon from San Juan to Miami on September 6, 2026. The 767 ran off the end of runway 30, hit navigation equipment and struck two vehicles. Five people in a passenger van were killed. Three more people on the ground were seriously hurt. The two pilots had minor injuries.

The preliminary report was released this week. It sets out facts only and does not assign a cause.

What the report says

Finding Detail
Speed over the threshold 177 knots
Touchdown 160 knots, 3,750 feet down a 9,360-foot runway
Stopping devices Speed brakes stayed stowed; thrust reversers did not deploy
Go-around Started 16 seconds after touchdown
Wind 190° at 19 knots, gusting 22; not given to the crew with the landing clearance
Crew experience Captain about 7,267 hours, 150 on the 767; first officer about 2,800 hours
Final report TBA

The first officer warned several times during the approach that the aircraft was too fast. The controller had read the wind to another aircraft four minutes earlier, before the 21 Air crew was on the frequency. Reports also note there was no arresting system at the end of runway 30. Amazon paused its flying with 21 Air after the accident.

What it means for non-revs

There is no standby angle here: 21 Air is a cargo operator. The report matters to airline people for other reasons.

  • Pilots and dispatchers will recognise the themes: an unstable approach, a late go-around decision, and a missed wind call from ATC.
  • Cargo crews and jump-seat riders should note Amazon’s pause with 21 Air. How long it lasts, and what it means for the airline’s flying and jobs, is TBA.
  • Expect follow-up on runway-end safety areas at Miami and on tailwind reporting procedures as the investigation continues.
